MY EXPERIENCES WITH GENERAL CELLULAR AND NEURAL CELLULAR PATHOLOGY IN A CASE BASED BLENDED LEARNING ECOSYSTEM'S CBBLE

MY FIRST PATIENT INTERACTION
A 35 year old male patient came to general medicine department with complaints of vomitings 3 days back. Till then I thought vomiting would be a common complaint for example as in fever but for the first time i have learnt how to differentiate it from other causes by detailed history taking as number of episodes, colour, contents. This first encounter made me realize the importance of taking detailed history for which I have to thank my professors.
In this case vomitings were associated with blood for which I have learnt the causes could be bleeding from esophagus/stomach/duodenum.on further questioning of patient about his daily activities, diet,sleep, addictions i got to know that he is occasional alcoholic since 10 years.As it was my first case I could only take upto the history.When I have gone through the investigation reports I have come to know that he is suffering from reflux esophagitis. Then I could relate between the causes of hematemesis and this patient.At that moment I realised that each and every history from the patient could lead us more near to the diagnosis.so history paves us the path to the diagnosis.I would really thank my professors for stressing us(students) on taking detailed history.
Next in the same patient when I learnt about the treatment given for this patient I have learnt the importance of contribution of Pharmacology from my 2nd year, as in this patient H.pylori kit was prescribed .with this I could remeber the drug therapy(Triple therapy/quadruple/sequential therapy) which I have learnt in my 2 year only for theoretical purpose. This made me realise that integration of our knowledge from each and every subject is essential and whatever we learn in our whole journey of MBBS is required to completely treat the disease. so I want to conclude by saying that detailed history taking is of prime importance for the sucessful journey of any medical aspirant throughout his career.I want to once again thank my professor and HOD sir of General medicine department for letting me realise the importance of history taking.
